REGISTRATION FORMS
Model Assembly Student Registration | A-1
This is the packet used to register all delegates. It contains biographical and medical information in addition to release forms and the Code of Conduct. Registration forms must be submitted to the local delegation by the registration deadline in order for students to participate. This form packet must be signed by the parent/guardian in order for a student to take part in the program.
Model Assembly Adult Registration | AD - 1
This is the packet used to register all adult advisors. It contains biographical and medical information in addition to release forms and the Code of Conduct. Registration forms must be submitted to the local delegation by the registration deadline. Adult advisors must complete a Volunteer Background Check prior to the conference.
Delegates who wish to apply for the Media program area (TV, Radio or Newspaper) must complete this form and accompany it with their registration form. No media assignments will be made without this form and attachments.
Personal Pricing Plan Application
Delegates who wish to apply for Personal Pricing must complete this form and all requested materials to be eligible for an award.
These forms are due with A-1 registration packet.

Model Assembly Appointed Official Form | B-1
Delelgates who wish to apply to be an Appointed Official need to completer this form and turn in to the Secretary of Statel Office at the Hotel by the last day of the conference.
AT THE CONFERENCE | FILING FORMS
Constitutional Filing Form | C-1
Delegates who wish to run for an Elected Officer position (Governor/Lt. Governor, Secretary of State and Attorney General)must complete this form and turn into the YIG State Office by Monday, November 28th, 2011 (5 PM Sharp). Late forms will not be accepted.
Program Area Filing Form | C-2
Delegates who wish to run for an Elected Officer Program Area Position (Speakers, Presidents, Judges, Lobbyist Director, NIF Presiding Officer or Media Directors) must turn in this form to the Secretary of State Hotel Office by Friday (12 Noon) during the Model Assembly conference.
Candidate Expenditure Form | C-3
Constitutional delegates must complete this form and turn in to the Secretary of State Hotel Office by Friday (12 Noon) during the Model Assembly conference to be eligible for candidacy.
